Request a Copy of Your Medical Records
Patients and legally authorized representatives may request copies of patient records maintained by Texas Tech Health El Paso. A completed and signed authorization is required before records can be released.
Authorization Forms
Select the appropriate form, complete all applicable fields and sign the authorization. The forms include instructions for identifying the records requested, the receiving party, the purpose of the release and the preferred format.
How to Submit a Request
Complete the authorization form.
Include the patient information, the person or organization that should receive the records, the specific records or dates of service requested, the purpose of the release and the requested format.
Sign the form.
The patient or a legally authorized representative must sign the authorization. The authorization form states that it expires 180 days after it is signed unless another expiration date or event is specified.
Provide proof of identity.
Be prepared to verify the identity and authority of the person making the request.
Submit the request to Medical Records.

Pay any applicable fee.
Copying, delivery or other fees may apply as permitted by law. Medical Records will provide information about any charge associated with the request.
Before You Submit
- Requests for psychotherapy notes require a separate authorization available on the HIPAA approved forms page.
- Select only the record types and dates needed when a complete medical record is not required.
- Provide complete recipient contact information to help avoid processing delays.
